You May Apply For a Passport Locally
The Clerk of the Superior Court is the local authorized United States passport acceptance agency for Ware County.
Who May Apply For a Passport
A U.S. passport may be issued to a citizen or national of the United States. Unless specifically authorized by a passport issuing office, no person may have more than one valid or potentially valid U.S. passport of the same type at any one time.
Applying For a U.S. Passport Locally
Submit the following to the Clerk of Superior Court in Ware County:
Completed passport application (DP-11) which may be obtained from the Clerk's Office.
Certified copy of birth certificate (has raised seal or impressed seal.) If you are a Georgia resident, and you do not have a certified copy of the birth certificate, you may contact the Ware County Board of Heath by telephone at 912-283-1875. The address for the Ware County Board of Health is 604 Riverside Drive, Waycross, Georgia 31501.
Two passport photos (2" X 2"), which can be obtained from Wal-Mart, Walgreen and Sears.
Proof of identity (picture identification, such as driver's license, etc.)
For children under the age of sixteen (16), both parents must sign the passport application, except in extenuating circumstances (more information will be provided about this topic upon contacting the the Clerk of Superior Court's Office.)
U.S. passports are issued only to U.S. citizens or nationals.
Each person must obtain his or her own passport. If you are a first-time applicant, please complete and submit this application in person. It is mandatory for children under sixteen (16) years of age to accompany parents. Each application must be accompanied by:
- Proof of U.S. citizenship.
- Proof of identity.
- Two (2) photographs.
- Fees.
